Adult Counseling

JFCS counseling and psychotherapy services are provided by licensed professionals who are equipped with the tools and resources to help individuals lead independent and fulfilled lives.  Individual, couple, and family therapy is available at convenient times at affordable fees. Sliding scale fees are available based on income. Third party insurances, including Magellan, Aetna, Medicare, and other third party health care providers, are accepted.

Emails will only be answered during regular business hours and may take up to 48 hours for a reply.

For all new clients who have scheduled their first appointment, please print and complete the form below and bring to your first office visit.  If you are unable to complete this form in advance, please plan to arrive at least 15 minutes prior to your first appointment.

In addition to the completed form, please also bring your insurance card (if applicable) to your appointment.  We will make a copy to include in your file. 


H1N1 Policies and Procedures

Generally, JFCS requires at least 24-hours notice for the cancellation of an appointment.  Appointments canceled with less than 24-hours notice are subject to a cancellation fee.  However, if you are experiencing influenza-like symptoms, your appointment may be canceled with less than 24-hours notice and will not be subject to the cancellation fee.

  • Everyone is encouraged to get vaccinated for seasonal influenza.  For information on groups prioritized for seasonal influenza vaccines, please click here.
  • Everyone is also encouraged to get vaccinated for 2009 H1N1 influenza when vaccines are available to them.  Different groups are prioritized for 2009 H1N1 influenza than for seasonal influenza.  For information on groups prioritized for H1N1 influenza vaccine, please click here.
